A Case report form (pronounced: /keɪs rɪˈpɔːrt fɔːrm/) is a document used in clinical trials to collect data in a standardized manner. The form is designed to capture all the necessary information about a patient's health status. The data collected is then used for statistical analysis and to draw conclusions about the safety and efficacy of a treatment or intervention.

The term "case report form" is derived from the English words "case", referring to an instance of a particular situation or issue, "report", meaning a detailed account or statement, and "form", indicating a document with spaces where information can be inserted.
