Medical Home

Medical Home (pronunciation: /ˈmɛdɪkəl hoʊm/), also known as Patient-Centered Medical Home (PCMH), is a model of primary care that is patient-centered, comprehensive, team-based, coordinated, accessible, and focused on quality and safety.

Etymology

The term "Medical Home" was first used by the American Academy of Pediatrics (AAP) in 1967. It was initially used to describe a central location for archiving a child's medical record. Over time, the concept evolved to represent a philosophy of health care that is patient-centered, comprehensive, and coordinated.

Definition

A Medical Home is not a building, house, or hospital, but rather an approach to providing comprehensive primary care. In a Medical Home, a personal physician leads a team of individuals who collectively take responsibility for the ongoing care of patients. The physician-led care team is responsible for providing all the patient's health care needs and, when needed, coordinates care across the health care system.
