Transitional care (pronunciation: tran-zish-uh-nl kehr) refers to the coordination and continuity of health care during a movement from one healthcare setting to either another or to home, between health care practitioners and settings as their condition and care needs change during the course of a chronic or acute illness.

Etymology

The term "transitional" is derived from the Latin word "transitio", which means "going across or over". The term "care" comes from the Old English word "caru", which means "sorrow, anxiety, grief", which later evolved to mean "charge, oversight, attention or heed with the purpose of doing something".
