List of terms related to Mesotherapy

Mesotherapy is the process of dealing with mesenchymal tissue such as excess fat tissue (unwanted fat), cellulite, and skin laxity.

Injection lipolysis

Treatments most properly called “injection lipolysis” have been commonly associated with the term “mesotherapy” to reduce or eliminate unwanted local accumulations of fat. Various terms for treatments that purport to “dissolve” fat seem to be used interchangeably, although “mesotherapy” has gained prominence in the public vernacular.


Injection lipolysis is typically done with trade-named products such as Lipodissolve™ and Lipostabil™ or with proprietary formulations provided by compounding pharmacies commonly called PCDC. The one common ingredient in all injection lipolysis formulations is phosphatidylcholine (PPC). In the United States, sodium deoxycholate (DC), a constituent of bile, is a second major ingredient used to keep the PPC soluble and in an injectable form without precipitating out of solution.

Phosphatidylcholine (PPC) and sodium deoxycholate (DC) are both phospholipids, emulsifiers, and surfactants. PPC is the most abundant phospholipid component of cell membranes, a precursor to acetylcholine, and a constituent of lipoproteins. DC is a constituent of bile. Both substances are naturally present in the human body.

In contrast to injections into the mesoderm, injection lipolysis treatments are delivered into the subcutaneous fat. In both cases, the depth of injection is critical to prevent damage to fascia. It has been hypothesized that treatment with PPC and DC reduces subcutaneous fat by adipocyte necrosis due to direct toxic or surfactant effects.

Kybella injections

The United States FDA approved a drug called Kybella for the treatment of excess chin fat or double chin with the active ingredients of deoxycholic acid. Each 2 mL vial of KYBELLA (deoxycholic acid) injection contains 20 mg synthetic deoxycholic acid as the active ingredient and the following inactive ingredients: benzyl alcohol (18 mg), dibasic sodium phosphate (2.84 mg), sodium chloride (8.76 mg), sodium hydroxide (2.86 mg) in water for injection, USP.


Tripollar Radio Frequency Lipolysis

The Tripollar Radio Frequency energy generates heat through tissue resistance in both the dermal and subcutaneous layers. Selective and focused electro-heating of the skin stimulates and increases fat metabolism in the subcutaneous fat layer, and collagen remodeling in the dermal layer. In the subcutaneous fat layer, the thermal effect accelerates the natural fat metabolism process and the release of liquid fat from the fat cells into the extra-cellular matrix. This fat then gets mobilized as liquid fat drained through the lymphatic channels and moves out of those areas leading to thinning of the fat layer as well as skin tightening as this process also stimulates new collagen formation. The tripollar radio frequency (RF) is considered a very effective and somewhat superior in terms of cost and effectiveness for localized fat reduction compared to cryolipolysis or cool sculpting as most tripollar RF sessions cost less than a third of what cool sculpting costs per session with comparable or better results.

Cryolipolysis or cool sculpting

Cryolipolysis is a method to remove fat by freezing. The method involves controlled application of cooling within the temperature range of -11 to +5 °C for the non-invasive, localized reduction of fat deposits, intending to reshape the contours of the body. The degree of exposure to cooling causes cell death of subcutaneous fat tissue, without apparent damage to the overlying skin. It appears primarily applicable to limited discrete fat bulges. Adverse effects include transient local redness, bruising and numbness of the skin are common side effects of the treatment and are expected to subside. Typically sensory deficits will subside within a month. The effect on peripheral nerves was investigated and failed to show permanent detrimental results.

Based on the premise that fat cells are more easily damaged by cooling than skin cells (such as popsicle panniculitis), cryolipolysis was developed to apply low temperatures to tissue via thermal conduction. In order to avoid frostbite, a specific temperature level and exposure are determined, such as 45 minutes at −10 °C (14 °F). While the process is not fully understood, it appears that fatty tissue that is cooled below body temperature, but above freezing, undergoes localized cell death ("apoptosis") followed by a local inflammatory response that gradually over the course of several months results in a reduction of the fatty tissue layer.

Typical cost per treatment area varies depending on location. Price in the US ranges from $750 to $1,500, with UK prices about £750 per area to be treated. Treatment time for general use/application is 35–60 minutes per site, depending on the applicator used. In the U.S., the CoolSculpting procedure is FDA-cleared for the treatment of visible fat bulges in the submental area, thigh, abdomen and flank, along with bra fat, back fat, underneath the buttocks, and upper arm. It is also FDA-cleared for use on submental area treatments.

Ultrasound lipolysis

Focused thermal ultrasound techniques work by raising the tissue temperature above 56 °C, resulting in coagulative necrosis of adipocytes, with sparing of vessels and nerves. Passive heating of the skin may also induce collagen remodeling.

Hydrolipoclasy is a technique that is being studied as an alternative to liposuction. It involves injecting a hypotonic solution into an area of fat and then subjecting it to ultrasound waves.

Low level laser light lipolysis

Low level laser light reduces the stability of adipocyte cell membranes, allowing cells to release their stores of fat without damaging the cell.
