Trueta began his career at the Hospital de la Santa Creu i Sant Pau in Barcelona, where he honed his skills in orthopedic surgery. His work gained prominence during the Spanish Civil War, where he developed the "Trueta method" for treating open fractures. This method involved meticulous cleaning of the wound, removal of foreign bodies, and the application of a plaster cast to immobilize the fracture, which significantly reduced infection rates and improved healing outcomes.

During World War II, Trueta moved to England, where he continued his work at the Nuffield Orthopaedic Centre in Oxford.
