Read Code
Read Code (pronounced: /ri:d koʊd/) is a coded thesaurus of clinical terms that has been used in the National Health Service (NHS) since the 1980s. It is named after Dr. James Read, who was the NHS's first GP IT advisor.
Etymology
The term "Read Code" is derived from the name of Dr. James Read, who developed the system. The word "code" refers to the alphanumeric system used to represent medical terms.
Description
Read Codes are a standard clinical terminology system used in General Practice and other areas of the NHS in the UK. They are alphanumeric and were designed to provide a standard vocabulary for clinicians to record patient findings and procedures, in health and social care IT systems across primary and secondary care.
Versions
There are two versions of Read Codes: Version 2 (5-byte set) and Version 3 (CTV3 or 5-byte set). Version 2 is the most widely used in general practice, and Version 3 is used in the Clinical Terms Version 3 (The Read Codes).
