Aggressive Angiomyxoma

Aggressive angiomyxoma (pronunciation: uh-gres-iv an-gee-oh-mix-oh-ma) is a rare, locally invasive tumor that predominantly affects the pelvic and perineal regions of females. Despite its name, it is not typically aggressive in the sense of widespread metastasis, but rather in its tendency to recur locally after surgical removal.

Etymology

The term "aggressive angiomyxoma" is derived from three words: "aggressive", referring to its tendency to recur; "angio", a prefix from the Greek angeion meaning "vessel", referring to the tumor's rich vascular component; and "myxoma", from the Greek muxa meaning "mucus", referring to the myxoid (mucous-like) background in which the tumor cells and vessels are embedded.

Clinical Presentation

Aggressive angiomyxoma typically presents as a painless, slowly growing mass in the pelvic or perineal region. It is most commonly diagnosed in women of reproductive age, but can occur in men and women of any age. The tumor can cause symptoms related to its size and location, such as urinary frequency, constipation, or discomfort during sexual intercourse.

Diagnosis and Treatment

Diagnosis of aggressive angiomyxoma is typically made by biopsy, with histopathology showing a characteristic pattern of spindle-shaped tumor cells and numerous small blood vessels in a myxoid background. Imaging studies such as MRI or CT scan can help to define the extent of the tumor and plan surgical treatment.

Treatment of aggressive angiomyxoma typically involves surgical removal of the tumor. Due to the tumor's tendency to recur, long-term follow-up is necessary. In some cases, hormone therapy may be used as an adjunct to surgery, as the tumor cells often express hormone receptors.
